Request for Periodicals

Periodicals are currently inaccessible due to renovations. Please use this form to request a periodical during our transition. After printing, please turn this form in at the Circulation Desk.

Please use a separate form for each item requested.

You will be expected to retrieve this item within 3 days of placing your request.
You will be notified only in the event that your request cannot be fulfilled.

Materials requested Monday - Thursday will be available 24 hours after the request was made. Materials requested Friday - Sunday will be available the following Monday afternoon. Materials will be held at the Circulation Desk for three days unless otherwise specified.
