Paperwork for your first appointment |

This page is provided for you to fill out paperwork in advance of your first
appointment. At your first appointment you will be asked to provide some basic
information about yourself on an Intake Form. Some students prefer to complete
this in advance and bring it with them at the time of their first meeting but
you may also complete in the office when you arrive if you prefer.
Please read our welcome brochure before you complete the intake form, as you will be required to sign that you have read the welcome brochure. If you don't have an appointment yet, please follow this link to make an appointment before completing this paperwork. Please bring the intake form to your first appointment. Do not send the intake form electronically or drop it off beforehand. Informed Consent for Minors - Students who are under the age of 18, need their parent or Guardian's' consent to participate in treatment.
