  If eligible for benefits, you must complete a Health Insurance Transaction form whether you are enrolling or declining coverage.  Choose the applicable form:

  PS404 For all employees EXCEPT Graduate Assistants
  If enrolling, before completing Box C of item 10, you should first read about the Pre-Tax Contribution Program.
  

  PS404g ONLY for Graduate Assistants
  If enrolling, before completing Box B of item 10, you should first read about the Pre-Tax Contribution Program.
