Evaluation, Final Internship Assignments and Forms
Assignments are to be completed at different intervals throughout your internship (see Internship Daily Logs)
EVALUATION
An evaluation
of students internship will be based on the following:
- Weekly E-mail Reports: The intern must report to the faculty supervisor by e-mail at least ONCE EACH WEEK during the internship period. FAILURE TO MAKE WEEKLY EMAIL REPORTS MAY BE CAUSE FOR RECEIVING A FAILING GRADE IN THE INTERNSHIP. The email report should be a brief statement regarding progress of the internship and the student's description of what is being learned. The purpose of the weekly email is to act as the sort of conversation or conference you might have if you were able to go to your faculty supervisor's office and talk about the internship. In your email, you'll probably discuss the various tasks you've been asked to do, as well as your impressions of what you're learning. Your daily logs will be more detailed and probably longer than your weekly email. Interns are required to check their email at least 3 times spaced throughout each week. All interns must have reliable access to email during the entire internship period. It is strongly suggested that interns DO NOT use their internship supervisor's server to send weekly internship email to the faculty supervisor. In order to ensure privacy interns should not send email to faculty from the internship site.
- Faculty Supervisor/Agency Consultation: A faculty supervisor will request a brief on-site consultation, or other consultation, with the internship agency supervisor to ascertain the student's responsibilities and progress in the internship. The faculty supervisor will also speak separately with the student about the internship experience.
- Internship Agency Evaluation: Near the end of the internship, the Music Department will send a one-sheet evaluation form to the on-site supervisor for comments and a recommendation for a final grade.
- Successful Completion of Required Contact Hours: Completion of 400 contact hours of approved internship/field experience (for students matriculated after Fall 2009, completion of 360 contact hours of approved internship/field experience.)

FINAL INTERNSHIP ASSIGNMENTS
Students need to complete & submit Daily Logs; Student Verification of Hours Worked Form, and Learning Outcomes Form (see details below for each).
- Internship Daily Logs:
Daily logs must be submitted in hard copy at TWO (2) times.
Daily Logs: Internship logs (for hours served up to time of supervisor visit) must be given to the faculty supervisor at the time of the supervisor's visit to the internship.

The logs must be typed or word-processed. The student will keep an accurate daily log of each day's work during the internship, including days, dates and hours worked.
- If an activity is performed more than once, it should be described in detail the first time and simply listed the second time. The first time that a student puts together a press packet, for example, the log entry might describe the contents of the packet and include whatever additional information seems appropriate. Example: "The press kit included an artist picture and bio, as well as press releases from the last major tour. It also included the name of a contact person, upcoming concert dates and record release dates. The press release was written by the Director of Promotion."
- Student Verification of Hours Worked Form:
The Student Statement of Hours Worked Form must be completed by the student showing a computation of total hours worked during the internship on a week by week basis. The form should be placed at the beginning of the daily logs when they are turned in at the end of the internship. The Statement of Hours must be SIGNED AND DATED by the student. The hours should be verifiable by examining the logs and by speaking with the intern supervisor.

- Learning Outcomes Form:
The Learning Outcomes Form should be completed and submitted with other required work at the conclusion of the internship.
*How to Submit Final Internship Assignments:
Submit final assignments by U.S. Mail to Music Department Office, 115 Fine Arts, Oneonta, NY 13820 if time permits. If not, submit Daily Logs by email to Music Department Secretary and fax final assignment forms to Music Department Office (607) 436-2718.
Required assignments must be received in by the Music Office upon completion of required internship contact hours and no later than the last day of classes for the semester in which the internship has been registered and completed.
The student will not receive a grade for the internship until the Music Department secretary has received the successfully completed assignments and the faculty supervisor has evaluated them.
Note: Please be aware that if you have applied for graduation and you have not finished your requirements by the deadline, you will need to resubmit your degree application and application fee to the Registrar's Office.
COURSE EVALUATION
The student evaluation of instruction is a valuable tool in ensuring the continued success of the internship program. Course Evaluations are NOT considered part of your Internship assignments nor are they used in the evaluation a student's internship.
Students are asked to fill out a "SPI" (Student Perception of Instruction Evaluation) form and free response comment sheet. The evaluation is not part of the student's internship assignment or grade.
Instructors will not see the results of student evaluation until after the internship grade has been submitted. The forms will be sent to students by the Music Department secretary. Students are asked to return them to the Music Department Office in the envelope provided. DO NOT return "SPI" forms to your faculty supervisor. |
