Student: Please print out this form and fill out the first four lines. Give it to the faculty member whom you wish to serve as your reference. You must have one recommendation for each academic department for which you wish to serve as a tutor. For example, if you want to tutor SPAN 101, SPAN 104, BIOL 100, MATH 173, MATH 174 and STAT 101, you need one form filled out by a Spanish professor you have had (for 101 or 104), one by the Biology 100 professor you had, and one by a math professor you have had (for Math 173, 174 & STAT 101).
